Volunteer Process Forms
FIELD TRIPS
FIELD TRIPS - STUDENTS
Field trips will be coordinated with State Standards for each grade level and arranged by individual teachers or teacher teams. Students MUST show good behavior and complete all school assignments in order to go on these trips. Field Trips are considered a privilege to enhance learning, they are not a right.
Students may not go on a field trip without a permission slip signed by a parent or guardian.
FIELD TRIPS - PARENTS
Volunteers are needed on most field trips. Family members interested in chaperoning field trips should contact individual teachers. Attempts are made by teachers to allow every interested volunteer an opportunity to accompany a group on a field trip. Adults are required to fill-out the same field trip permission form above.
MEDICATIONS
If a child must take prescription medication of any kind at school, please follow these steps:
- Complete a medication release form (downloadable PDF below).
- Medication brought to school must be in a labeled pharmacy container with the child’s name, doctor’s name, dose and instruction for use clearly printed.
- The Administrative Secretary and Office Technician will dispense medication.
- Over the counter medication (aspirin, Tylenol, cough drops, throat lozenges, antihistamines, cold medications, inhalers, etc.) will only be given if prescribed by a doctor and must be in writing on a medical release form.
- Do not send medication to school in a child’s backpack, lunch box or pocket.

INTENT TO RETURN
Every year, current students have the first opportunity to re-enroll at CVC. Ample notice is given to families about re-enrollment. “Intent to Return” forms are typically sent home in January and due back to the school office in February.
If an “Intent to Return” form is not turned in by the designated deadline noted on the form, the student will lose his/her place at CVC and will be required to enter the lottery process. NO EXCEPTIONS.
